As an NDIS Support Coordinator, you will be at the heart of this mission. You will work alongside participants, their families, carers, and support networks to help them understand and implement their NDIS plans, connect with quality service providers, overcome barriers, and build the skills and confidence to manage their supports independently where possible.

Key Responsibilities
- Support participants to understand and implement their NDIS plans, ensuring services align with their individual goals and needs.
- Build positive, trusting relationships with participants, families, carers, and support networks.
- Identify, source, and coordinate suitable service providers that meet NDIS Practice Standards and deliver high-quality, person-centred supports.
- Advocate for participants to ensure they receive the services and supports they need to achieve their goals.
- Monitor service delivery, resolve issues proactively, and assist participants to change providers where required.
- Work collaboratively with allied health professionals, support workers, plan managers, and community organisations to coordinate effective supports.
- Assist participants to prepare for plan reassessments and reviews by gathering supporting evidence and identifying changing support needs.
- Monitor budgets and service utilisation to maximise participant outcomes while ensuring funding is used effectively.
- Develop contingency plans to minimise disruptions to essential supports and services.
- Maintain accurate case notes, reports, and documentation in accordance with NDIS requirements and organisational policies.
- Build participants' capacity to make informed decisions, strengthen their independence, and increase their confidence in navigating the NDIS.
About You
We're looking for someone who is compassionate, organised, proactive, and solutions-focused. You will be an excellent communicator who enjoys working collaboratively and is committed to delivering exceptional outcomes for participants.
You will have:
- Experience in NDIS Support Coordination, Case Management, Community Services, Disability, Mental Health, or a related field.
- A strong understanding of the NDIS and person-centred practice.
- Excellent organisational, problem-solving, and time management skills.
- The ability to build genuine relationships with participants and stakeholders.
- Strong written communication and report-writing skills.
- A current Driver Licence, National Police Check, NDIS Worker Screening Check, and Working with Children Check (or the ability to obtain them).
