About the Role
You will help NDIS participants understand and implement their plans, connect with appropriate providers, manage their funding and work towards their individual goals.
Weekly hours are not fixed and will depend on the active caseload, billable hours, participant demand and achievement of agreed KPIs.
Key Responsibilities:
- Help participants understand and implement their NDIS plans.
- Develop, coordinate and review person-centred support arrangements.
- Connect participants with suitable providers and community supports.
- Monitor participant funding, service agreements and plan utilisation.
- Liaise with families, plan managers, allied health professionals and providers.
- Complete intake documentation, case notes and progress reports.
- Build referral networks and contribute to participant-base growth.
- Meet agreed caseload, reporting, compliance and billable-hour KPIs.
Essential Requirements
- Minimum two years’ Support Coordination or relevant sector experience.
- Sound knowledge of the NDIS and participant plans.
- Existing disability-sector connections and referral networks.
- Strong communication, organisation and caseload-management skills.
- Ability to work independently in a hybrid environment.
- Valid driver’s licence and access to a reliable, appropriately insured vehicle.
- Current NDIS Worker Screening Check and other required clearances.
- Current First Aid and CPR certificates.
- Australian working rights.
Hours and KPI Expectations
Hours will be based on participant demand, active caseload, billable Support Coordination hours and achievement of agreed performance expectations.
KPIs may include participant onboarding, caseload growth, timely documentation, service quality, participant retention and development of referral relationships.
