Kanda is seeking an enthusiastic
Home Care Packages (HCP) Coordinator to join our Veterans’, Aged Care, and Community Nursing Services team in our Spring Hill Office. This full-time role supports the coordination and delivery of person-centred care, ensuring clients receive high-quality services while maintaining compliance with Aged Care Quality Standards.

Your role
- Coordinate and oversee Home Care Package services for clients
- Support clients to understand and achieve their care goals
- Build positive relationships with clients, families, and key stakeholders
- Manage client onboarding, reviews, and ongoing service changes
- Maintain compliance with Aged Care Quality Standards and documentation requirements
- Work closely with the Senior Clinical Nurse to support quality service delivery
- Respond to client feedback, incidents, and continuous improvement initiatives
- Contribute to the growth and development of the Home Care Package program
What we are looking for
- Proven experience in a similar role within aged care, community care, or disability services
- Relevant qualification in Health, Community Services, or similar experience
- Strong communication and stakeholder engagement skills, with a compassionate and client-focused approach
- Excellent organisational, time management, and attention to detail
- Ability to manage competing priorities in a fast-paced environment
- NDIS Worker Screening Clearance, or willingness to obtain one
