We're looking for a compassionate, organised, and proactive Care Coordinator to join our growing Adelaide team. This is an opportunity to become one of the founding team members of our South Australian office and help shape the experience our Members receive from day one.

What You'll Be Doing
- Supporting Members and their families throughout their care journey.
- Coordinating services and referrals to ensure support is implemented promptly.
- Communicating with Members, families, suppliers, and healthcare professionals.
- Monitoring service delivery and following up on outstanding actions.
- Maintaining accurate records and documentation within our systems.
- Supporting Care Managers to ensure Members receive timely and appropriate services.
- Assisting with the implementation of support hours and service schedules.
- Helping coordinate Helpers and services to ensure Members receive reliable support.
- Acting as a key point of contact for Members and helping resolve issues with empathy and professionalism.
About You
- Strong organisational and communication skills.
- Experience in aged care, community services, health care, rostering, administration, customer service, or care coordination is highly regarded.
- Ability to build genuine relationships with Members, families, and colleagues.
- Comfortable managing multiple priorities and solving problems independently.
- Strong attention to detail and confidence using technology and computer systems.
- A positive, proactive, and team-focused attitude.
