Where you'll be working
Based at Macquarie Hospital and working across Northern Sydney LHD community health centres.
What you'll be doing
Working within the experienced physiotherapy and multi-disciplinary team at CDCRS (Chronic Disease Community Rehabilitation Service). Co-ordinating and conducting group exercise programs for outpatients with chronic respiratory conditions and/or chronic heart failure. Undertaking individual patient assessments, developing evidence-based personalised rehabilitation plans, monitoring patients before, during and after exercise.

Candidates will need to meet the following criteria:

Full registration with the Physiotherapy Board of Australia. Evidence of extensive clinical experience with a minimum at a Level 2 Physiotherapist.

Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement.

Demonstrated proficient skills and knowledge in the provision of current physiotherapy practice. A strong commitment to an evidence-based service to provide for consumers, carers and families requiring assessment, case management, intervention, appropriate referral and discharge planning.

Unrestricted NSW Drivers licence.
