We're excited to welcome a Pharmacist to our Pharmacy department.
The common objective of the Pharmacy Service is that patients derive maximum benefit from medications whilst being exposed to minimum risk. This purpose will be achieved by the deployment of a skilled and caring pharmacy workforce.
Key responsibilities:
Deliver general level clinical pharmacy services throughout the patient journey within a rostered clinical area or in the dispensary
Documenting patient admissions (inclusive of a best possible medication history), conducting clinical medication reviews, and collaboratively formulating medication management plans.
Collaborate with other healthcare professionals, ensuring a holistic and multidisciplinary approach to patient care within the hospital.
Provide accurate and timely advice to medical, nursing and subordinate pharmacy staff on patient admission history and pharmaceutical issues while ensuring optimum patient medication safety.
Remuneration value up to $10,162 p.m., comprising salary between $3,067.00 - $4,592.80 p.f., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (HP3) (fixed term temporary position available for up to 12months).
