Join our Ambulatory Allied Health Service Team
Support hospital admission avoidance and early supported discharge for Southern Adelaide Local Health Network (SALHN) hospitals whilst also providing discipline specific support to SALHN Chronic Disease Pathways. This role is based at SALHN GP Plus sites (primary site location flexible).
About the role:
Reporting to the Ambulatory Allied Health Team Manager, this role is responsible for delivering culturally appropriate, equitable, comprehensive, high quality and contemporary integrated care focused physiotherapy services for patients in the Ambulatory Allied Health Service.
Duties for the role will include:
Providing physiotherapy assessment that enables appropriate evidence based physiotherapy interventions such as functional rehabilitation, exercise prescription, gait re-education and prescription of appropriate mobility and transfer aids
Physiotherapy intervention and prescription of exercises to prevent falls through improved balance and mobility.
Planning and coordinating services including those involving other disciplines or agencies as required to meet individual health care needs
Contributing to the strategic planning for physiotherapy services across relevant work areas, incorporating regular evaluation of services and reporting on quality improvement activities
Working collaboratively within a multidisciplinary team, including contributing professional physiotherapy expertise input and facilitating complex multi-disciplinary service delivery
Supporting AHP1/AHA staff and students allocated to the work area to contribute to high quality and timely service delivery, and to assist them in prioritising their clinical responsibilities and workload
Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.
About you:
Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
Demonstrated experience in the delivery of relevant models and practices within a community based, ambulatory and/or Integrated Care framework
Utilise high level communication skills to effectively work with, and resolve issues and conflict with, people with diverse value systems, cultural differences and special needs, including clients, multi-disciplinary health professionals, other organisations and providers, and the community.
Undertake a range of evidence based physiotherapy assessments and interventions on an individual and group basis that support self-management, independence and safety in the community
Organise workloads, under limited direction in a high-demand multifaceted environment and as part of a multi-disciplinary team
Critically and reflectively evaluate own work, and maintain own professional development to meet requirements
Apply a high level of professional accountability, integrity, ethical standards and decision making, and to be self-aware and positive when faced with difficult clinical and interpersonal situations.
Use professional judgement in clinical decision making, which contributes to the development of new and innovative service models to respond to the changing needs of the health system and the community
