As the Registered Nurse, you will:
Provide comprehensive, coordinated and person-centered nursing care to individuals and families within their homes and community settings. Provide a client goal focused service to empower them to achieve their health goals
Delivering a broad range of services including chronic disease management, wound and catheter care, palliative care, health prevention, health promotion and career support. Post acute services such as CVAD management, VAC management, Chemotherapy disconnections, drain management
You will work collaboratively with consumers, carers, General Practitioners and multidisciplinary teams to ensure continuity of care and optimal health outcomes.

About You
Our ideal candidate will demonstrate:
Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
Experience or interest in community nursing, including chronic disease management and post-acute care
Strong communication and interpersonal skills with the ability to build therapeutic relationships with individuals, families, carers and multidisciplinary teams
Ability to conduct comprehensive assessments, develop and evaluate person-centered care plans, and deliver safe, evidence-based nursing care in community and home-based settings
