Servicing in all regions of Melbourne, Permalink is rapidly growing and expanding its care services in the Northern, Western, Southern, and Eastern regions of Melbourne, supporting more than 500 people each year!
We are seeking a dedicated and compassionate GROUP ACTIVITIES COORDINATOR to join our team Monday-Friday 9:00AM-5:00 PM. The ideal candidate will have a genuine interest in improving the lives of individuals with disabilities and providing high-quality support services and 1:1 support to our participants.
Our Group Activities Coordinators contributes to the achievement of servicing those who require immediate access to services and ongoing rosters of care. Services include providing personal care, assistance with domestic tasks, and facilitating community access.
You will be hired directly by Permalink as an employee and not subcontracted. (TFN only, not ABN)
The Group Activities Coordinator responsibilities will include and not limited to:
- Coordinate and facilitate engaging, safe, and person-centred group activities that align with participants' individual NDIS goals, interests, abilities, and support needs.
- Consistently attend scheduled weekly programs, community outings, team meetings, and work sessions, ensuring reliable service delivery and a positive participant experience.
- Ensure all services are delivered in accordance with the NDIS Practice Standards, NDIS Code of Conduct, organisational policies, and relevant legislative requirements.
- Promote participant choice, independence, dignity, and social inclusion while encouraging skill development, confidence, and community participation.
- Respond promptly and professionally to participant concerns, incidents, behavioural escalations, and complaints, ensuring appropriate support, documentation, and escalation in accordance with organisational procedures.
- Complete accurate incident reports, progress notes, risk assessments, and other required documentation within designated timeframes.
- Liaise effectively with the Permalink Rostering Team, Service Specialists, Support Coordinators, participant plan nominees, families, carers, advocates, and participants' next of kin to ensure continuity of care and accurate service delivery.
- Review and interpret participant NDIS Plans, Care Plans, Behaviour Support Plans, risk assessments, and health information to ensure supports are delivered safely and in accordance with participant goals.
- Monitor participant attendance, engagement, outcomes, and progress towards individual goals, providing feedback and recommendations where appropriate.
- Identify emerging participant needs and communicate changes promptly to relevant internal teams to ensure supports remain responsive and person-centred.
- Coordinate and support Masters of Social Work placement students by planning learning opportunities, providing orientation, facilitating supervision, and monitoring placement objectives.
- Attend placement meetings, supervision sessions, and stakeholder meetings, ensuring compliance with university and organisational placement requirements.
- Facilitate group supervision and reflective practice sessions for Masters of Social Work students, promoting professional development and evidence-informed practice.
- Mentor, coach, and provide guidance to new staff, students, and volunteers to support quality service delivery and organisational values.
- Relay critical participant updates, schedule changes, risk alerts, and service delivery information to the Rostering Team and relevant stakeholders in a timely manner.
- Collaborate with multidisciplinary teams, allied health professionals, and external service providers to coordinate participant supports and improve outcomes.
- Assist with planning, scheduling, resource allocation, transport coordination, venue bookings, and logistical requirements for group programs and community activities.
- Monitor staffing requirements and provide recommendations to support appropriate participant-to-staff ratios and safe program delivery.
- Ensure all group activities comply with Work Health and Safety (WHS) requirements, infection prevention and control standards, and organisational risk management procedures.
- Conduct dynamic risk assessments before and during activities, implementing appropriate controls to maintain participant and staff safety.
- Support medication administration processes (where authorised) and respond appropriately to medical emergencies following organisational procedures.
- Foster an inclusive, culturally safe, and participant-centred environment that respects diversity, individual preferences, and human rights.
- Build positive relationships with participants, families, carers, community organisations, and external stakeholders to enhance engagement and community inclusion opportunities.
- Contribute to quality assurance initiatives, audits, continuous improvement activities, and the review of organisational policies and procedures.
- Maintain confidentiality and manage sensitive participant information in accordance with privacy legislation and organisational requirements.
- Prepare reports, collect outcome data, and contribute to program evaluations to measure participant satisfaction and service effectiveness.
- Participate in professional development, mandatory training, and ongoing learning to maintain current knowledge of NDIS legislation, disability practice, safeguarding, and best-practice service delivery.
- Support the development of innovative programs and activities that promote participant wellbeing, capacity building, independence, and meaningful community participation.
- Demonstrate leadership by modelling organisational values, promoting teamwork, resolving day-to-day operational issues, and contributing to a positive workplace culture.
We are seeking a highly driven and motivated individual with strong verbal and written communication and relationship-building skills. The right person for our team will possess strong problem-solving skills and someone who can exhibit a can-do attitude.
Please note that experience in the NDIS industry or any healthcare industry is a must!
Qualifications and experience:
-Must have a Masters of Social Work or Bachelors of Social Work with a minimum 2 years post industry experience in Social Work or Case Management
-AASW Accreditation (preferred)
-Must have evident experience in Disability Care and the NDIS sector
-Pleasing, friendly, bubbly and outgoing personality
-Previous experience in Group Programs and Facilitations
-Fluent and strong verbal and written communication

-NDIS Worker Orientation Module Certificate

Pay: $37.84 per hour
