Help Older People Stay Independent, Mobile and Living Well at Home

We're looking for a full-time Community Physiotherapist to support older people living in their own homes across Melbourne. Working with Members receiving Support at Home (SAH) funding and transitioned Home Care Packages (HCP), you'll help people maintain their mobility, confidence, independence, and quality of life for as long as possible.This is a community-based role focused on meaningful outcomes — helping people stay active, reduce falls risk, improve strength and balance, recover from illness or hospitalisation, and continue doing the things that matter most to them.You'll have the opportunity to build genuine relationships with Members, collaborate closely with Care Managers and families, and see the real impact of your work over time.

What You'll Be Doing
- Conducting physiotherapy assessments within Members' homes
- Supporting mobility, strength, balance, and falls prevention
- Developing practical home exercise programs
- Helping Members maintain independence and confidence in daily activities
- Supporting recovery following illness, injury, hospitalisation, or health decline
- Providing recommendations regarding mobility aids and equipment
- Collaborating with Care Managers, GPs, family members, and other allied health professionals
- Monitoring progress and adjusting interventions as Member needs change
- Contributing to a coordinated approach to care that helps Members live safely and well at home
About You
- AHPRA Registered Physiotherapist
- Current driver's licence and reliable vehicle
- Strong communication and relationship-building skills
- Passionate about helping older people maintain their independence
- Comfortable working autonomously in a community setting
- Experience in aged care, community health, rehabilitation, or home-based services is highly regarded

Pay: $80,000.00 – $90,000.00 per year
