About the role
Permanent part time opportunity to provide transdisciplinary occupational therapy and physiotherapy screening, assessment, intervention, and care coordination of patients presenting to Royal Brisbane and Women's Hospital (RBWH) Internal Medicine and Aged Care (IMAC) services to enhance the patient flow.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide clinical transdisciplinary assessment, intervention, and discharge planning of frail, elderly patients, in partnership with the existing multidisciplinary team, to support patient flow through timely access to allied health services and facilitate discharge particularly on weekends. Core clinical tasks of this transdisciplinary role include functional assessment of mobility and ADLs (transfers, showering, toileting, dressing), upper and lower limb neurological / musculoskeletal assessments, prescription of mobility aids and assistive equipment, pressure area and positioning management, and cognitive assessment using standardised tools.
Develop and maintain strong internal and external networks and work collaboratively and effectively across multiple professions, services, and stakeholders within and external to the RBWH to achieve seamless transition of acute medical patients through the continuum of care.
Collaborate with senior management and other key service stakeholders on issues relating to the MFIT Service, clinical operations, service needs and to assist in shaping the model's overall future strategic direction.
Utilise a high degree of organisational awareness, and research and evaluation skills, to contribute to the ongoing monitoring, evaluation, and redesign of the scope of practice of the Allied Health Coordinator role, and the model of service, against proposed outcomes and identified key performance indicators.
