About the role
Reporting to the General Manager and working in close partnership with Community Leaders and the care and clinical teams, the Lifestyle Coordinator ensures lifestyle practice aligns with Rockpool's governance framework, the Aged Care Act and the Strengthened Aged Care Quality Standards, including assessment, care planning and documentation obligations relating to each resident's social, emotional, cultural and spiritual wellbeing.
You are skilled at drawing out each resident's story and translating it into individualised lifestyle planning, building a program residents are proud of and championing resident choice every day so the program genuinely reflects the My Day My Way model of care, rather than a one-size-fits-all activities calendar.
Key responsibilities include:
- Design, coordinate and continuously improve a monthly lifestyle calendar of group and individual activities that reflects residents' interests, cultures and abilities
- Complete lifestyle assessments and social profiles for each resident on admission and at regular reviews, and develop individualised lifestyle care plans in consultation with residents and families
- Provide program direction, coaching and professional guidance to the Lifestyle Officers embedded within each care team, partnering with Community Leaders to ensure consistent, high-quality engagement across seven days
- Build and maintain community partnerships, including schools, clubs, artists, faith communities and local organisations, and develop intergenerational and community-facing programs
- Plan and deliver events, celebrations and culturally significant days that bring the home together
- Partner with the clinical and allied health teams so that lifestyle programming complements the home's reablement and wellness program
- Identify residents at risk of social isolation and ensure meaningful one-to-one engagement is planned and delivered
- Ensure activities are inclusive, dignified and adapted for residents living with dementia or with sensory, communication or mobility needs
- Manage the lifestyle budget, resources and supplier relationships responsibly and creatively
Skills and experience
- Demonstrated experience designing and delivering lifestyle programs for older people, including residents living with dementia
- Sound knowledge of the Strengthened Aged Care Quality Standards and person-centred assessment and care planning
- Strong documentation and computer skills
- Demonstrated event planning and community engagement capability
- Excellent communication and relationship-building skills with residents, families, colleagues and community partners
- Resident-first, collaborative and values-led, bringing genuine enthusiasm, warmth and empathy to your work every day
- Creative and influencing, generating fresh programming ideas and building credibility with Community Leaders and care teams without relying on direct line authority
- Organised, able to plan and coordinate a busy calendar of people, spaces and events so the program runs reliably across seven days
- A Certificate IV in Leisure and Health (or Diploma or equivalent), or substantial experience in lifestyle, leisure or wellbeing roles in aged care or community services
