About the role
Temporary full time opportunity to apply your professional knowledge and clinical skills to address the complex social, emotional, behavioural and environmental factors that impact health and recovery. You will work collaboratively with patients, families, healthcare professionals and community services to facilitate safe, patient-centred care, informed decision-making and effective discharge planning.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Deliver competent evidence based clinical services, including counselling, in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards through assessing the complex psychosocial needs and risks of patients/families/carers.
Work collaboratively and effectively with other social work practitioners, professional and operational supervisors, relevant service managers and other stakeholders regarding social work service delivery.
Provide clinical practice supervision, education and support to less experienced practitioners and students to ensure the maintenance of clinical standards.
Actively contribute to a designated portfolio within the Social Work Service, contributing to quality improvement, research and innovation activities.
