About the Role - As a Care Partner, you will coordinate services, develop individualised care plans and provide ongoing care management to ensure participants receive high-quality, person-centred services aligned with the Support at Home Program. You will build trusted relationships with participants and their families while working collaboratively with internal teams and external health professionals to deliver outstanding outcomes.
Your responsibilities will include:
- Conduct participant assessments and develop personalised care plans
- Monitor, review and evaluate participant goals and changing care needs
- Coordinate services with Community Support Professionals and external providers
- Build strong relationships with participants, families and health professionals
- Monitor participant budgets in accordance with Support at Home guidelines
- Identify emerging risks and escalate clinical concerns where appropriate
- Advocate for participants and support informed decision-making
- Ensure documentation is accurate, compliant and completed within required timeframes
- Participate in quality improvement initiatives and maintain compliance with aged care and NDIS practice standards
About You
You are compassionate, organised and passionate about providing exceptional customer service and person-centred care.You enjoy building relationships, solving problems and working collaboratively with others to achieve the best outcomes for participants.
To be successful you will have:
- Qualifications in Individual Support (Ageing), Health Services Assistance, Aged Care, Disability, Community Services, Nursing or equivalent (i.e. Certificate III in Individual Support (Ageing) or Certificate IV in Aged Care) or willingness to work toward desired qualifications
- Experience in a Care Partner, Case Management or Service Coordination role
- Experience within Aged Care, Community Care, Disability or Health Services
- Strong organisational and time management skills
- Excellent written and verbal communication skills
- The ability to coordinate multiple participants and competing priorities
- Experience working collaboratively within a multidisciplinary team
- Strong computer and administration skills
- Current Driver's Licence
- Current NDIS Worker Screening Check (or willingness to obtain)
