This role offers the opportunity to make a real difference for people living with chronic and complex health needs by helping them stay well in the community and avoid unnecessary hospital visits. As a Clinical Nurse Specialist Grade 2 in the Keeping Well in the Community (KWIC) Care Navigation and Care Coordination team, you will provide senior clinical leadership and work closely with clients, families, and service partners to navigate complex health and psychosocial needs. Using a person‑centred, mostly telephone‑based model, you will assess, coordinate, and problem‑solve care across hospital, community, and primary care settings.
This role is vital to supporting timely facilitated discharge, reducing avoidable presentations and admissions, and improving continuity of care for some of the district’s most vulnerable people. You will also contribute to quality improvement, support clinical decision‑making within the team, and help shape how integrated care is delivered across the service.By joining KWIC CNCC, you will be part of a team committed to safe, compassionate, and coordinated care, making a meaningful impact on individual lives while strengthening patient flow and sustainability across the health system.
Are You the Right Fit?

You are an experienced and confident Registered Nurse with strong clinical judgement and a genuine commitment to improving outcomes for people with complex and chronic health needs.

You bring advanced nursing knowledge, sound decision‑making skills, and the ability to work autonomously while staying connected to a multidisciplinary team.

You are skilled in clinical assessment, care navigation, and coordination, and are comfortable managing complexity, prioritising risk, and escalating concerns appropriately.

You communicate clearly and compassionately, building strong relationships with clients, families, and service partners across hospital, community, and primary care settings.

You value person‑centred, inclusive care, are reflective in your practice, and contribute positively to team culture through collaboration and respect.
You are motivated, organised, and able to adapt in a dynamic service environment, while supporting quality improvement and safe clinical practice.
