Where you'll be working
The WSLHD Dementia Advisory Service operates across multiple sites within Western Sydney Local Health District, partnering with community service providers to deliver integrated support for people living with dementia, their families and carers. You will be based in the Geriatric Medicine Department at Westmead Hospital – a specialist hub for complex aged care services.
The Geriatric Medicine Department supports a broad spectrum of aged care services including the RACE program, Geriatric Day Hospital, outpatient clinics and community-based services.
Work at the intersection of clinical practice and community development, engaging with NDIS, ACAT, aged care facilities and non-government organisations.

What you'll be doing
The Level 3 Senior Social Worker has a recognised clinical specialty in the area of dementia and provides high-quality clinical services to clients with dementia, their families and carers in the community, with a key role in community development and working in partnership with service providers.
Provide comprehensive Dementia Advisory services at sites across WSLHD including care planning, referral coordination, support groups, and education for clients, families, carers and community services.
Lead and contribute to quality improvement activities, service planning and the implementation of evidence-based clinical practice in collaboration with internal and external stakeholders.
Provide education consultations to Medical, Nursing and Allied Health staff and external agencies through in-services, seminars, case conferences and patient care meetings.
Provide supervision to Social Work students on clinical placement in accordance with AASW guidelines, and prepare for and seek regular supervision per AASW and departmental guidelines.
Maintain responsibility for personal and professional development through evidence-based practice activities, training, clinical supervision and performance reviews.

Selection Criteria
Bachelor of Social Work and eligibility for membership of the Australian Association of Social Work (AASW), and a current Driver's licence.
Demonstrated clinical experience in Dementia care.
Experience in community development and working in partnership with services providers.
Demonstrated experience in the provision of staff and/or student supervision.
Experience in developing and implementing policies and procedures for service improvement and skills in providing training and education.
Well-developed time management skills with excellent written and verbal communication, including report writing skills.
Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
Demonstrated experience in quality assurance and/or clinical practice improvement activities.
