NDIS Disability Support Worker – Daily Living & Community Access Support
Position: Disability Support Worker
Location: Coolbellup
Hours: 6 hours in home support per day ( 0900 earliest -4pm latest ) and 4 days of community support for 3 hours

About the Participant
We are seeking a compassionate, reliable, and experienced Disability Support Worker to support a participant living with the effects of a stroke. He experiences significant physical, cognitive, and communication challenges and requires consistent support to achieve his NDIS goals of increasing independence, participating in the community, and maintaining his health and wellbeing.
Key Responsibilities
Daily Living Support (6 Hours Daily)
* Assist with personal care and daily living activities as required.
* Support safe mobility and transfers in accordance with therapy recommendations.
* Monitor positioning of the right upper limb to minimise the risk of injury and shoulder subluxation.
* Support meal preparation and healthy lifestyle routines.
* Assist with medication prompts and health management routines where required.
* Encourage participation in therapeutic exercises and activities recommended by allied health professionals.
* Support fatigue management and pacing strategies throughout the day.
* Assist with maintaining a safe home environment.
Community Access Support (3 Hours)
* Facilitate participation in social, recreational, and community activities.
* Support attendance at appointments, shopping, and community programs.
* Assist to navigate community environments safely, particularly in busy settings where attention and visual challenges may impact safety.
* Encourage social engagement and development of confidence and independence.
* Support implementation of strategies recommended by therapists to improve participation and community access.
Essential Requirements
* Certificate III or IV in Individual Support, Disability, Community Services, or equivalent.
* Experience supporting individuals with acquired brain injury, stroke, neurological conditions, or complex disabilities.
* Current NDIS Worker Screening Check.
* Current Working with Children Check (if required).
* Current First Aid and CPR Certificate.
* Australian Driver’s Licence and reliable vehicle with comprehensive insurance.
* Strong understanding of person-centred support and positive risk management.
* Ability to follow therapy recommendations and support plans consistently.
Desired Skills
* Experience supporting participants with cognitive impairments and neurological conditions.
* Knowledge of fatigue management strategies.
* Strong communication and documentation skills.
* Ability to work independently while maintaining participant safety.
