Country Connect Foundation is a First Nations-led not-for-profit organisation committed to delivering culturally grounded Specialist Homelessness Services, Supported Crisis Accommodation and Outreach Support in remote Aboriginal communities across the Northern Territory. About the Role
We are seeking a compassionate reliable Case Manager to join our Short Stay Homelessness Program. This role is to manage our Outreach service stream in Nhulunbuy, and provide additional support to our Accommodation service stream. Our Short Stay Homelessness Program supports First Nations adults who are experiencing, or at risk of, homelessness - providing a safe, calm, and culturally respectful environment during a time of transition.
Working as part of a committed team, you’ll deliver trauma-informed, strengths-based care that responds to each person’s unique needs. You’ll assist with daily routines, emotional support, and access to services, while maintaining a safe and welcoming space.
This position is based in Nhulunbuy, please be advised that housing is not provided as part of this role.

Key Responsibilities
Oversee outreach case management practices and support sustainable housing pathways for individuals and families experiencing or at risk of homelessness
Provide direction and support to case workers delivering outreach across Nhulunbuy and surrounding communities
Identify and engage individuals experiencing housing instability, including those who may not otherwise be accessing homelessness services
Work alongside the Accommodation Case Manager to actively manage the Short Stay Accommodation waitlist, maintaining regular engagement and providing support to individuals while they await accommodation
Provide person-centred outreach support to address immediate needs, reduce risks and work towards individual housing and wellbeing goals
Support individuals with housing applications, advocacy, referrals and engagement with housing and accommodation providers
Connect individuals with health, AOD, mental health, financial, legal and other essential services, maintaining follow-up where required
Work closely with the Accommodation service stream to coordinate pathways into and beyond Short Stay Accommodation
Build and maintain strong relationships with Aboriginal organisations, housing services, government agencies and other community partners
Maintain accurate case notes, assessments, referrals and program records and contribute to program reporting requirements
Identify and respond appropriately to risk, vulnerability and crisis situations, escalating concerns where required
Identify emerging homelessness trends, service gaps and unmet needs to support responsive service delivery
Embed local cultural leadership and ensure outreach is culturally safe, trauma-informed, inclusive and responsive

About You
Essential Criteria:
Understanding of trauma-informed care and the importance of cultural safety
Comfortable supporting people with complex needs or challenging circumstances
Reliable, calm, and respectful in your approach
Excellent communication and interpersonal skills.
Ability to work independently and as part of a cross-cultural team.
Ability to collaborate with stakeholders to deliver integrated support.
Valid NT Driver’s License.
Working with Children Check (Ochre Card).
NDIS Worker Screening Check (or willingness to obtain).
First Aid & CPR Certificate (or willingness to obtain).
Desirable Criteria:
Experience working in housing, homelessness, or community services desirable
Certificate III or IV in Community Services, Mental Health, AOD, or a related field
Understanding of First Nations cultures and communities is highly regarded
4WD driving skills and confidence in off-road travel.
