We are currently seeking experienced, motivated, and compassionate Support Coordinators to join our growing team, with opportunities available in the Newcastle/Gosford and Campbelltown/Macarthur regions.
This is a full-time hybrid position, offering the flexibility to work from home while maintaining meaningful face-to-face engagement with participants and community stakeholders.
About the Role :
As a Support Coordinator, you will play a pivotal role in helping NDIS participants navigate their supports, build capacity, and connect with services that align with their goals and aspirations. You will work closely with participants, families, service providers, and allied professionals to ensure positive outcomes and lasting impact.
This role is ideal for someone who is passionate about advocacy, enjoys building strong relationships, and thrives in a dynamic and rewarding environment.
Key Responsibilities:
- Coordinate and manage NDIS-funded supports to ensure participants receive services that meet their individual goals and needs.
- Assist participants to understand and implement their NDIS plans effectively.
- Develop strong relationships with participants, families, carers, and key stakeholders.
- Connect participants with suitable service providers and community supports.
- Monitor participant progress and proactively identify opportunities to improve outcomes.
- Advocate for participants and support informed decision-making.
- Collaborate with internal teams, allied health professionals, and external providers to deliver high-quality, person-centred services.
- Maintain accurate records, case notes, and documentation in line with NDIS requirements and organisational standards.
- Support participants through changes, challenges, and transitions while promoting independence and capacity building.
What We're Looking For:
- Previous experience as a Support Coordinator, Case Manager, Recovery Coach, or a similar role within the NDIS, community, health, or disability sector.
- Strong knowledge of the NDIS and person-centred practice.
- Excellent communication, relationship-building, and stakeholder engagement skills.
- Exceptional organisational and time-management abilities.
- A proactive and solution-focused approach to problem-solving.
- Genuine passion for supporting people with disability to achieve their goals.
- Ability to work independently while contributing positively to a collaborative team environment.
- Current Australian Driver's Licence and access to a reliable vehicle.
- Relevant qualifications in Social Work, Community Services, Disability, Psychology, Human Services, or a related field will be highly regarded.
