We are seeking a full-time Community Worker to join our team in Adelaide, SA.
Your responsibilities will include:
Interviewing clients to understand their needs and assess difficulties they face
Preparing plans for clients advising on support programs and implementing all aspects of the
plan, including informal, mainstream, community, and funded support.
Following up with clients and monitor their progress and offer ongoing support and
guidance to participants to ensure their goals are met
Referring participants to appropriate service providers and support families by providing
education on available services in the community.
Assessing needs of the wider community for access to resources such as health, welfare,
housing, employment and other facilities.
Liaising with community agencies about issues and promoting services/resources offered
Preparing submissions and various reports for management and other agencies, including
implementation, progress, and end-of-plan reports.
To apply for the role, you must have the following:
Diploma level qualification OR 3 years of work experience in lieu.
You must also have at least 1 year of work experience in the field
The salary for the role is $79,500.
