About the role..
An exciting opportunity exists for an experienced and passionate
Senior Social Worker to join the NSW Spinal Outreach Service (SOS). In this rewarding role, you’ll provide holistic, client-centred support to individuals living with spinal cord injury and complex disabilities, helping them navigate rehabilitation and community living.
Working within a collaborative multidisciplinary team, you’ll use your therapeutic, clinical and case management expertise to support clients across a diverse geographical area. You’ll build strong relationships with clients, families and healthcare providers while managing a dynamic caseload and contributing to education, service development and quality improvement initiatives.
Supported by experienced Social Workers and reporting to the Manager of the NSW Spinal Outreach Service and Social Work Professional Leader, this role offers the opportunity to make a meaningful impact in a specialised outreach setting, including occasional travel to rural NSW to extend vital support to regional communities.
Key responsibilities include:
- Provide comprehensive social work assessment, case coordination and ongoing support to clients with spinal cord injury and their families/carers.
- Develop and implement person-centred goals and intervention strategies that support long-term community integration and independence.
- Liaise with healthcare providers, community organisations and external agencies to coordinate services, advocate for client needs and improve access to supports.
- Deliver education, consultation and emotional support to clients, carers, community providers and multidisciplinary team members.
- Participate in case conferences, interdisciplinary planning and discharge coordination to ensure integrated, high-quality rehabilitation outcomes.
- Contribute to service development, quality improvement, reporting and community engagement initiatives that enhance spinal outreach services across NSW.
- Comply with Royal Rehab and Social Work (SW) policies and procedures.
About you..
We seek a self-motivated and friendly individual with:
Essential Criteria:
- Tertiary qualifications in Social Work & eligible for membership of the Australian Association of Social Workers.
- Extensive clinical experience working with people with complex disability across a variety of settings.
- Extensive experience and demonstrated advanced level skills in counselling/casework, and advocacy.
- Experience working with individuals, families, and groups.
- Demonstrated ability to communicate effectively - both written and orally
- Ability to work independently and proven commitment to working effectively within a team framework.
- Extensive knowledge of community-based services – across Australia and specifically NSW.
- Demonstrated ability to develop and deliver formal education sessions to professionals, community workers and families.
- Well-developed case management experience and, or a solid understanding of the roles and functions within a multi-disciplinary team in a rehabilitation, outpatient, or community environment.
- Moderate to high level of computer literacy.
- Ability to travel periodically in rural NSW (approx. 3 times/year, for 3-4 nights duration on each occasion).
- Current Class C unrestricted NSW Driver’s License.
Desirable Criteria:
- Experience working with people with a Spinal Cord Injury.
- Experience in a community-based setting.
- Experience working with clients covered under insurances schemes and/or knowledge of the systems involved, and knowledge of NDIS.
- Experience working with people with a Traumatic Brain Injury.
