Role purpose:
To deliver person-centered onboarding services to new and prospective clients. The role is also responsible for providing high-quality and effective support the Support at Home (SaH) clients, Commonwealth Home Support Program (CHSP) clients and NDIS clients. Additionally, the role includes promoting and raising the profile of AVWA’s SaH, Commonwealth Home Support Program (CHSP) and NDIS Programs within the community, responding promptly to enquiries, explaining program requirements and costs, and supporting clients throughout their assessment and onboarding journey until services are successfully established.
What we are looking for:
1. Client Intake & Consultation
- Receive and respond to initial enquiries for SaH, CHSP, and NDIS services.
- Contact clients promptly to provide program information, consultation, and answers to enquiries.
- Counsel clients to obtain consent and collect necessary information, including family circumstances, medical conditions, care needs, Medicare details, and address.
- Enter accurate client information into intake records.
- Organise an Appointment of a Representative form when required.
2. Assessment Coordination
- Request and coordinate comprehensive assessments or reassessments when needed.
- Submit referrals and review requests to My Aged Care (MAC).
- Support clients to maintain clear communication with MAC after referrals are submitted.
- Liaise with Regional Assessment Services (RAS), Aged Care Assessment Services (ACAS), and Allied Health professionals.
- Follow up with clients throughout the assessment process until a final outcome is issued.
3. Onboarding & Service Establishment
- Follow up with clients until a service package or agreement is signed with AVWA.
- Explain AVWA’s service delivery model, fees, and charges in alignment with SaH, CHSP, and NDIS requirements.
- Advise approved CHSP clients on how and where to access care services.
- Monitor ADS assignments for approved clients.
- Support clients to complete Income Tested Care Fee forms if required.
- Assist clients with initial service setup to ensure timely commencement without disruption.
4. Program Promotion & Community Engagement
- Promote and raise awareness of AVWA’s SaH, CHSP, and NDIS programs within the community.
- Maintain positive relationships with external stakeholders and referral agencies.
The above list is not exhaustive, and the role may change to meet the overall objectives of AVWA. Any other relevant duties as reasonably required by the management.
Who you are:
Essential:
- Diploma in Community Services or other relevant qualifications.
- Experience in customer service.
- Solid understanding in CHSP, SaH and NDIS programs
- Strong communication, empathy and cultural competence
- Current Police Check or NDIS Worker Screening Check, and full COVID-19 vaccination
- Current First Aid/CPR certificates.
- Good English and Vietnamese communications.
