ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.
We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP.
Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive. Having the same support workers providing services, in particular, medication prompts and personal care, maintains the person’s dignity and well-being. You know and can trust the people attending. There is continuity of care for people who receive our services.

About the role:
The Clinical Care Partner will bring a clinical focus to our person-centred care model, supporting elderly clients to achieve their goals and maintain independence. This role includes acting as a liaison with clients, families, caregivers, and health professionals while offering clinical insights and support to the team. Your responsibilities may include:
- Supporting our Care Partners with clinical and care support for our complex, and high needs clients
- Ensuring services meet internal and external quality standards
- Driving innovation and continuous improvement aligned with ECHO’s strategic objectives
- Offering guidance and support to clients, families, and team members to deliver safe, effective care
Hours: Monday to Friday, 8:00 am to 4:00 pm. You will also be required to participate in a rotating on-call after hours, weekday and weekend roster.

Qualifications:
- Registered Nurse with a tertiary qualification in Nursing
- AHPRA registered
Essential:
- Minimum 2 years’ experience in a case management role within aged care or community care
- Demonstrated experience in providing clinical assessments, wound care, medication administration, continence management, chronic disease management (including diabetes), palliative and dementia care.
- Proactive approach to problem-solving
Competencies:
- Demonstrated organisational and time management skills
- Above average written and verbal communication skills
- Customer/ Client focus with a talent for building relationships and achieving positive outcomes
- Attention to detail and accuracy
- Fluent written and spoken English
- Initiative
Desirable:
- Management of SAH budgets
- Provide clinical care as needed
- Knowledge of Aged Care Quality Standards
- Understanding of Clinical Governance and Risk Management
