Allegiance Heart & Home Care is seeking an experienced and compassionate Physiotherapist to provide ongoing weekly physiotherapy services to an older person receiving care at home.
This is a great opportunity for a physiotherapist looking for a consistent weekly client, flexible scheduling and meaningful community-based work focused on maintaining mobility, independence and quality of life.
The physiotherapist will provide one home-based physiotherapy visit each week, with ongoing service.
The role will focus on assessing and supporting the client's mobility, strength, balance, functional capacity and falls risk while working toward practical goals that help the client remain safe and independent at home.
Responsibilities may include:

Conducting physiotherapy assessments and ongoing reviews.
- Developing and implementing an individualised physiotherapy treatment plan.
- Supporting improvements in mobility, strength, balance and endurance.
- Providing appropriate falls-prevention strategies.
- Reviewing gait, transfers and functional mobility.
- Providing therapeutic exercises appropriate to the client's abilities and goals.
- Monitoring progress and adjusting the treatment program where required.
- Providing education to the client and family/support persons regarding safe mobility and exercise.
- Identifying any deterioration, emerging risks or additional allied health needs.
- Recommending appropriate mobility aids or equipment where clinically indicated.
- Liaising with the client's Care Manager and other health professionals where required.
- Completing clear and timely clinical documentation following each appointment.
We are looking for a physiotherapist who is professional, reliable and comfortable providing services independently within a client's home.
The successful applicant should have:

Current AHPRA registration as a Physiotherapist.
- Current professional indemnity and public liability insurance where applicable.
- Current National Police Check.
- Current driver's licence and reliable transport.
- Experience working with older people, rehabilitation, mobility, falls prevention or community care highly regarded.
- Strong clinical assessment and treatment-planning skills.
- Excellent communication and interpersonal skills.
- Ability to work independently while communicating effectively with the care-management team.
- Strong documentation and clinical reporting skills.
- A respectful, person-centred and collaborative approach to care.
