About the Role
As a Support Worker with Resilience Community Care, you will provide individualised support that enables participants to achieve their goals, maximise independence and actively participate in their community.
Every day is different, and your role may include:
- Personal care including showering, dressing, grooming and medication assistance in accordance with care plans and training.
- Domestic assistance including cleaning, laundry and meal preparation.
- Community access including appointments, shopping, social activities and recreational outings.
- Supporting participants to develop independence and life skills.
- Safe use of mobility equipment including wheelchairs, hoists and transfer equipment.
- Supporting participants with complex care needs and behaviours of concern in accordance with Behaviour Support Plans and Positive Behaviour Support strategies.
- Monitoring participant wellbeing and promptly reporting changes or concerns.
- Completing accurate, objective and professional shift notes and incident reports using our electronic systems.
- Working collaboratively with participants, families, allied health professionals and the wider Resilience Community Care team.
About You

- Flexible with availability across a variety of shifts.
Essential Requirements
- Experience working within the disability support sector.
- Current First Aid and CPR Certificate.
- Current National Police Check.
- Working with Children Check (Blue Card).
- Current NDIS Worker Screening Clearance (Yellow Card).
- Current Australian Driver Licence.
- Reliable, roadworthy motor vehicle with comprehensive insurance.
- Smartphone with reliable internet access.
- Current right to work in Australia.
Desirable
- Certificate III or IV in Individual Support, Disability or equivalent.
- Experience supporting participants with mental health and/or psychosocial disability.
- Experience supporting participants with high-intensity or complex support needs.

Availability
Resilience Community Care operates 24 hours a day, 7 days a week.
Applicants must be available to work a variety of shifts, including:
- Day shifts
- Afternoon and evening shifts
- Active overnight shifts
- Sleepover shifts
- Weekend shifts
- Public holidays
