About Boian Night Chemist
Since 1970, Boian Night Chemist has provided extended-hours care and individualised attention to the health needs of customers in Sydney's St. George area.
Our dedicated pharmacists are available as an initial point of reference in emergency medical situations, to advise on personal health concerns, provide specific medication information and dispense prescriptions. Boian Night Chemist operates extended hours daily from 9:00am to 10:30pm.
Building on our longstanding commitment to pharmacy and community healthcare, we are expanding our involvement in residential aged care through the Aged Care Onsite Pharmacist (ACOP) Program.
About the ACOP Role
As an Aged Care Onsite Pharmacist, you will work onsite within a residential aged care home and collaborate closely with the home's clinical and care teams.
The role provides an opportunity to use your clinical pharmacy knowledge to contribute directly to medication management, medication safety and quality improvement initiatives within the aged care environment.
Depending on the needs of the service and residents, your responsibilities may include:
- Providing onsite clinical pharmacy support and medication-related advice;
- Supporting medication reviews and quality use of medicines;
- Identifying and helping address medication-related risks and opportunities;
- Working collaboratively with Registered Nurses, Care Managers, GPs/prescribers and other healthcare professionals;
- Supporting residents and families with medication-related information and education;
- Contributing to medication safety initiatives, audits and quality improvement activities;
- Supporting medication reconciliation and transitions of care;
- Providing medication-related education to aged care employees where appropriate;
- Participating in relevant clinical governance and multidisciplinary activities; and
- Maintaining appropriate clinical documentation and records associated with ACOP activities.
About You
We are looking for a pharmacist who is professional, approachable and comfortable working collaboratively within a multidisciplinary healthcare environment.
You will ideally demonstrate:
- Hold current AHPRA registration as a Pharmacist;
- Be appropriately credentialed to provide Aged Care Onsite Pharmacist (ACOP) services;
- Be appropriately credentialed to provide Medication Management Review (MMR) services;
- Be able to communicate professionally in Chinese (Mandarin and/or Cantonese) to support effective communication with residents and other relevant stakeholders within the aged care environment;
- Have strong clinical pharmacy and medication management knowledge;
- Have a genuine interest in residential aged care and improving outcomes for older people;
- Demonstrate strong communication and interpersonal skills;
- Be able to establish effective professional relationships with GPs, Registered Nurses, Care Managers, residents, families and pharmacy colleagues;
- Have good organisational and time-management skills;
- Be capable of working independently within the residential aged care environment while remaining connected with the broader pharmacy and care teams; and
- Demonstrate sound professional judgement and a commitment to medication safety and quality use of medicines.
Previous experience in residential aged care, medication management reviews, clinical pharmacy or aged care pharmacy services would be highly regarded.
