About the role:
Join the Murrumbidgee Local Health District as a clinician within the Community Transition Aged Care Program (TACP), where you will play a vital role in helping elderly clients safely return home from hospital. Working as part of a collaborative multidisciplinary team in Wagga Wagga and surrounding areas, you will conduct comprehensive assessments, develop personalised care plans, and deliver both direct and virtual care. Our ideal candidate will:

Hold relevant tertiary qualifications as an Allied Health Professional or Registered Nurse with full AHPRA registration, a minimum of three years clinical experience, and demonstrated experience working with elderly clients in aged care settings.

Possess sound knowledge of the aged care system, including My Aged Care, Transition Care Programs, Aged Care Assessment Service (ACAS), and Out of Hospital Care Programs.

Demonstrate strong written and verbal communication skills and the ability to work both independently and collaboratively within a multidisciplinary team, including in virtual care environments.

Show a proven ability to implement virtual care technologies and models of care to improve client access and outcomes, while thinking critically to manage complex clinical situations in a rapidly changing environment.
