Michael Vasili Chemist is an independent pharmacy that has specialised in providing quality pharmaceutical services to aged care facilities across NSW for over 25 years.
We are looking to expand our pharmacy team and offer accredited pharmacists the opportunity to work in the aged care sector as part of a clinical multidisciplinary team.
Role Overview:
As an ACOP, you will play a pivotal role in enhancing the quality of life for residents through:
- Person-centred care: Providing personalised medication management and addressing inquiries from residents, families, carers, and healthcare professionals.
- Interprofessional collaboration and communication: Working closely with multidisciplinary teams to ensure optimal medication use and safety.
- Medication management: Conducting medication reconciliation, facilitating facility-wide quality improvement activities, and managing drugs of dependence.
- Quality Use of Medicines (QUM): Conducting clinical audits, reviewing RACF medication management policies, and ensuring continuity of care.
- Clinical governance: Setting up and actively participating in medication advisory committees, assessing and responding to medication-related incidents, and assisting with mandatory quality indicator reporting.

A Successful Candidate Must Have:
- Accreditation for medication management review services, with intent to complete Aged Care Accreditation once available.
- Recognised undergraduate pharmacy degree or equivalent, with unconditional AHPRA registration.
- Australian citizenship or permanent residency status.
- Appropriate police clearance.
- Effective communication skills, both written and verbal, for interacting with residents, families, and healthcare professionals.

Pay: $120,000.00 per year
