About this Opportunity
Join the Mental Health in Aged Care (MHiAC) program and make a positive difference in the lives of older people living in Residential Aged Care Facilities across Gippsland. Developed by Wellways, MHiAC provides face-to-face, telehealth and telephone-based mental health support for residents experiencing mild to moderate mental health concerns. Working under the guidance of the Senior Practitioner, you will deliver wellbeing sessions, support intake and assessment processes, manage referrals, provide active waitlist support, and contribute to positive recovery-focused outcomes for older people.
This role is ideal for someone with experience in mental health, aged care, community services or a related field who enjoys working collaboratively with clients, families, Residential Aged Care Facilities and community partners. You will help strengthen service pathways, coordinate access to appropriate supports, maintain accurate client records and promote person-centred, recovery-oriented practice. Current valid Driver's License and the ability to undertake travel for the role

Current Working with Children Check and NDIS Workers Screening Check
