As a Care Manager in our Horsham office, you will be responsible for coordinating and managing care services for prospective and existing clients, ensuring a high-quality and consistent client experience. You will lead the end-to-end client journey including onboarding, care planning and ongoing reviews, supporting clients to live as independently as possible at home.
You will work closely with caregivers, families and internal teams to ensure services are aligned to client needs, compliant with standards and delivered to a high standard.

Key responsibilities

Manage and coordinate care for prospective and existing clients
-
Develop, implement and review individualised care plans aligned to client goals and needs

Build strong relationships with clients and their families as a key point of contact

Conduct client onboarding, assessments and regular care reviews

Collaborate with internal teams to ensure effective caregiver matching

Maintain accurate client records and documentation

Ensure compliance with Support at Home guidelines and Aged Care Quality Standards

Coordinate with external providers to support holistic care delivery

Identify opportunities to improve care delivery and client outcomes
About you
You are an organised and client-focused professional who is passionate about delivering high-quality care outcomes. You bring a proactive approach, strong communication skills and the ability to manage multiple priorities in a fast-paced environment.
You'll also have:

Relevant qualification in Nursing (preferred)

Experience in care management, case management or a similar role is desired

Understanding of aged care or Home Care Packages/Support at Home
- Strong stakeholder and relationship management skills

High attention to detail and strong administrative capability

Ability to work both independently and as part of a team
