Are you a compassionate and experienced Clinical Nurse looking to make a meaningful difference? Join our Aboriginal Primary Health Care Service and become part of a dedicated multidisciplinary team committed to improving health outcomes for Aboriginal and Torres Strait Islander people.
Working alongside Aboriginal Health Practitioners, General Practitioners, specialist and allied health professionals, you will deliver high quality, culturally safe, person centred wholistic care that supports individuals and families.
About the role:
Provide nursing and/or midwifery services in a primary health care setting as a clinical nurse you will play a key role in providing comprehensive primary health care service within a culturally safe and supportive environment, working collaboratively with the team to empower community members to achieve better health outcomes and contribute to closing the gap.
You will provide high quality, culturally safe, person-centred clinical care across the lifespan.
You will work collaboratively with Aboriginal Health Practitioners, General Practitioners allied health professional and community members to deliver comprehensive primary health care that empowers individuals, families and communities.
Must be a compassionate and motivated Clinical Nurse committed to closing the gap in health outcomes for Aboriginal and Torres Strait Islander people.
Nursing assessments and care planning, chronic disease management, immunisations, health promotion and screening activities.
Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
Provide proficient, person centred, clinical nursing/midwifery care and/or individual case management to patients/clients in a primary health care setting.
Assists and supports the Regional Manager Clinical Services or equivalent in management, clinical, and education activities.
Participate in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience.
About you:
Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience.
Experience in the leadership and direction of less experienced registered nurses.
Experience in working closely with Aboriginal Health Workers and mainstream service providers.
