We are seeking an experienced Registered Nurse to join a growing healthcare team supporting young people within a youth justice setting.
As the Clinical Care Coordinator, you will play a critical role in coordinating healthcare services, managing care plans, supporting discharge planning, and ensuring young people receive seamless, high-quality care throughout their journey and transition back into the community.

The Clinical Care Coordinator is responsible for overseeing the coordination of healthcare services for young people in custody, from admission through to discharge and community reintegration.
Working closely with nursing, allied health, mental health, medical practitioners, youth justice staff, and external healthcare providers, you will ensure continuity of care, facilitate access to services, and advocate for the health needs of young people.
You will also play a key role in care planning, service improvement initiatives, stakeholder engagement, and clinical governance activities.

Coordinate healthcare assessments and ongoing care planning.

Facilitate access to medical, allied health, and mental health services.

Review and maintain individual healthcare plans.

Support coordinated discharge planning and continuity of care.

Advocate for young people to ensure equitable access to healthcare services.

Participate in multidisciplinary case discussions and care reviews.

Work closely with internal healthcare teams and youth justice stakeholders.

Coordinate referrals and specialist appointments with external providers.

Facilitate healthcare handovers and transitions between services.

Build strong relationships with community health providers to support successful reintegration outcomes.

Contribute to clinical audits, quality improvement initiatives, and service development projects.

Monitor healthcare outcomes and identify opportunities for improvement.

Support compliance with clinical governance, infection control, and risk management frameworks.

Assist in developing streamlined referral pathways and service delivery processes.

Maintain accurate and timely clinical documentation.

Ensure compliance with legislative, professional, and organisational requirements.

Support reporting and quality assurance activities.
To be successful in this role, you will bring:
- Current AHPRA registration as a Registered Nurse.

Minimum 5 years post-graduate clinical experience.

Strong experience in care coordination, case management, or complex healthcare settings.

Excellent clinical assessment and triage skills.

Demonstrated experience working with vulnerable, marginalised, or at-risk populations.

Outstanding communication, stakeholder engagement, and relationship-building skills.

Ability to work effectively within multidisciplinary teams.

Strong organisational skills with the ability to manage competing priorities.

Experience working within youth justice, corrections, custodial healthcare, community health, mental health, or alcohol and other drug (AOD) settings.

Experience supporting young people with complex psychosocial needs.

Postgraduate qualifications in health management, healthcare leadership, or a related discipline.
