Role description
An exciting opportunity exists for an experienced Registered Nurse to join Dhelkaya Health's Community Nursing team. This position provides high-quality nursing care to clients in their homes through District Nursing Services and Community Palliative Care programs. Working across a seven-day service, you will deliver expert, person-centred care, support health education, advocate for clients, assist with goal achievement and collaborate with healthcare providers to ensure coordinated care and positive outcomes.
Key Responsibilities
- Deliver high-quality, person-centred nursing care to community clients through District Nursing Services, Community Palliative Care and Hospital in the Home programs.
- Conduct comprehensive assessments and develop goal-focused care plans in partnership with clients and carers.
- Collaborate effectively with multidisciplinary teams and external healthcare providers to ensure continuity of care.
- Participate in case conferences, team meetings and service development activities.
- Maintain accurate clinical documentation and meet all reporting requirements.
- Support quality improvement initiatives and accreditation activities.
- Provide student supervision and contribute to a positive learning environment.
- Actively participate in professional development, performance review and clinical supervision programs
About you
To be successful in this role, you will have:
- Bachelor of Nursing with current AHPRA registration.
- Minimum two years full-time equivalent experience as a Registered Nurse.
- Current Victorian Driver Licence.
- Strong communication and interpersonal skills with the ability to build positive relationships with clients, carers and colleagues.
- Demonstrated ability to undertake complex assessments and develop person-centred care plans.
- Excellent organisational skills and the ability to effectively manage competing priorities.
- A flexible and adaptable approach with a commitment to collaborative, client-focused care.
- An understanding of community nursing and contemporary community health service delivery practices will be highly regarded.
