Client Support & Care Coordination
- Build positive and respectful relationships with clients, families, and carers.
- Conduct non-clinical client assessments and regular wellbeing reviews.
- Develop and maintain individualised support plans aligned with client goals and preferences.
- Coordinate and schedule home care services, support workers, and community supports.
- Monitor client satisfaction and respond promptly to concerns or service changes.
- Assist clients in navigating available community, aged care, and disability support services.
Service Delivery & Administration
Maintain accurate client records, progress notes, and service documentation.
Ensure services are delivered in accordance with organisational policies and procedures.
Support onboarding of new clients and assist with service commencement processes.
Collaborate with rostering and operational teams to ensure continuity of care and support.
Assist with incident reporting and follow-up actions when required.
Communication & Relationship Management
Act as a key point of contact for clients and families.
Liaise with support workers, coordinators, external providers, and community organisations.
Promote a positive client experience through responsive and compassionate communication.
Quality & Compliance
Support compliance with:
Aged Care Quality Standards
Work Health and Safety requirements
Organisational policies and procedures
Participate in continuous improvement initiatives and feedback processes.
3. Qualifications
Current Criminal Record Check
First Aid certificate and CPR
Current Australian Driving License
4. Competencies and experience
Experience in home care, aged care, community care, disability services, or care coordination.
Understanding of:
Home Care Packages (HCP) -Support at Home Program
Experience using care management or rostering systems.
Knowledge of culturally safe and inclusive care practices.
*Experience working with culturally diverse communities, including Aboriginal and Torres Strait Islander peoples, older persons, people with disability, or vulnerable individuals
Desirable
- Experience in assessment and care planning.
- Experience in Dementia and / or Palliative care.
- Knowledge of Aged Care Quality and Safety Standards.
- Familiarity with legislation and regulations related to aged care.
Experience as a care Partner is Mandatory
Pay: $38.50 – $40.50 per hour
