About the Role
You will support NDIS participants experiencing complex circumstances or significant barriers. This includes coordinating multidisciplinary supports, managing risks and strengthening the participant’s support environment.
Weekly hours are not fixed and will depend on caseload complexity, billable hours, participant demand and achievement of agreed KPIs.
Key Responsibilities
- Coordinate complex supports and address barriers affecting participants.
- Develop, implement and review complex service arrangements.
- Manage risks, service breakdowns and stakeholder concerns.
- Liaise with families, clinicians, government agencies and NDIS providers.
- Monitor participant funding, service agreements and plan utilisation.
- Facilitate case conferences, intake meetings and plan reviews.
- Complete detailed reports, case notes and risk documentation.
- Build referral networks and meet agreed caseload and performance KPIs.
Essential Requirements
- Minimum two years’ relevant experience, including high-level or specialist support coordination.
- Experience supporting participants with complex needs and risks.
- Strong knowledge of the NDIS and relevant regulatory requirements.
- Relevant qualification in social work, psychology, allied health, community services or a related discipline.
- Established disability, health or community-sector connections.
- Strong assessment, report-writing and stakeholder-management skills.
- Valid driver’s licence and access to a reliable, appropriately insured vehicle.
- Current NDIS Worker Screening Check and other required clearances.
- Current First Aid and CPR certificates.
- Australian working rights.
Hours and KPI Expectations
Hours will be based on participant demand, caseload complexity, billable hours, and achievement of agreed performance expectations.
KPIs may include participant onboarding, caseload growth, timely reporting, risk management, participant retention, service quality and referral-network development.
