MMR Connect links GPs with Accredited Pharmacists to deliver Home Medicines Reviews (HMR). We are seeking a dedicated and clinically focused Accredited Pharmacist to deliver high-quality HMR services to our local community.
In this role, you will play a vital part in patient safety and health outcomes. You will visit patients in their homes to review complex medication routines, ensuring therapies are safe, effective, and understood. You will work collaboratively with local GPs to create personalised medication management reviews, specifically for patients with polypharmacy or those recently discharged from hospital.
KEY RESPONSIBILITIES
- Conduct home visits for Medicare and DVA cardholders eligible for HMR services.
- Perform comprehensive medication reviews, identifying out-of-date, unnecessary, or potentially harmful medicines.
- Assess patient understanding and adherence to medication regimens.
- Draft detailed clinical reports for referring GPs to assist in medication management and collaborate together for best patient health care outcomes.
- Educate patients on the proper use of their medicines to reduce the risk of side effects or errors.
- Maintain accurate records in compliance with Australian Government funding requirements (PPA).
REQUIREMENTS
- Current Registration with AHPRAH
- Current HMR Accreditation (Essential).
- Strong clinical knowledge regarding polypharmacy and chronic disease management.
- Excellent communication skills with the ability to build rapport with patients in a home setting.
- Reliable vehicle and valid driver's license for home visits.
- Ability to work autonomously and manage your own schedule.
