This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.
It is a community health centre based position requiring travel across sites and into private residences. This position is required to participate in afternoon, weekend and public holiday shifts.
The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

What you'll be doing
As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and evidence-based clinical interventions to clients in the community. Working both independently and collaboratively, you will help clients and their families navigate complex health needs while supporting positive health outcomes and continuity of care.
- Conduct holistic nursing assessments and develop individualised care plans
- Coordinate care and provide clinical interventions, monitoring and ongoing support
- Educate and empower clients, families and carers to better manage their health conditions
- Collaborate with multidisciplinary teams to deliver safe, integrated and person-centred care
- Maintain accurate clinical documentation and contribute to quality and safety initiatives

- Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)
