We are looking for an Administration & Care Coordinator to join our growing team. This is a hands-on and varied position where you will be an important part of the day-to-day delivery of high-quality home care services.
You will work closely with clients, families, support workers, nurses, allied health professionals and external providers to ensure services are coordinated efficiently and our clients receive the support they need.

About the Role:
As an Administration & Care Coordinator, you will provide essential administrative and coordination support to ensure the smooth operation of our home care services.
You will be responsible for maintaining accurate client information, coordinating services and rosters, responding to enquiries and supporting our Care and Operations teams.
This role is ideal for someone who enjoys variety, problem-solving, organisation and working with people.
Key Responsibilities:
- Provide day-to-day administrative and coordination support to the Greater Parramatta team.
- Be a friendly and professional first point of contact for clients, families, support workers and providers.
- Respond to telephone and email enquiries promptly and professionally.
- Coordinate client services and ensure appointments and supports are accurately scheduled.
- Assist with staff rostering, shift changes, cancellations and replacement arrangements.
- Maintain accurate client and employee information across relevant systems.
- Assist with client onboarding, service agreements, referrals and documentation.
- Prepare and maintain client files, care plans, schedules and other required documentation.
- Assist with Support at Home administration, funding information and service coordination.
- Liaise with support workers, nurses, allied health professionals, trades and external service providers.
- Follow up outstanding actions, referrals, reports and documentation.
- Assist with invoicing, timesheets and other operational administration as required.
- Support compliance requirements, audits and quality documentation.
- Assist with incident, complaint and feedback follow-up where required.
- Maintain office records, supplies and general administration.
- Prepare reports, spreadsheets and correspondence using Microsoft Office.
- Identify opportunities to improve administrative and operational processes.
- Provide general support to the management and care coordination team as required.
About You
You are someone who takes ownership, stays organised and genuinely enjoys helping others.
You will ideally have:
- Previous experience in administration, customer service, care coordination, rostering or a similar role.
- Experience in aged care, home care, disability or community services is highly desirable.
- Knowledge of Support at Home, Home Care Packages (HCP) and/or NDIS is an advantage.
- Excellent communication and interpersonal skills.
- Strong attention to detail and accuracy.
- Excellent time management and organisational skills.
- Ability to manage competing priorities in a fast-paced environment.
- Confident telephone and customer service skills.
- Strong Microsoft Office skills, particularly Word, Excel and Outlook.
- Experience with CRM, client management or rostering systems is desirable.
- Strong written communication and documentation skills.
- Ability to work independently and as part of a collaborative team.
- A proactive, positive and solutions-focused attitude.
- High level of confidentiality and professionalism.
- A genuine passion for supporting older people and their families.

Essential Requirements
Successful applicants will be required to have, or be willing to obtain:
- Australian National Police Check
- NDIS Worker Screening Check
- Working with Children Check, where applicable
- Current First Aid & CPR
- Relevant vaccination requirements in accordance with organisational and client requirements
- Australian working rights
A current Australian driver's licence and access to a reliable vehicle will be highly regarded.
- Experience with Support at Home or Home Care Packages.
- Rostering experience.
- Experience working with care management or CRM software.
- Understanding of aged care compliance and documentation requirements.
- Experience coordinating multiple service providers.
- A Certificate IV or Diploma in Community Services, Aged Care, Business Administration or a related field.
