About the role
The Transitional Aged Care Program (TACP) Allied Health Assistant will carry out delegated tasks in community-based settings under the guidance and support of TACP multidisciplinary allied health team members comprising of Clinical Care Coordinators and Allied Health clinicians including Occupational Therapists, Physiotherapists, Social Workers, Speech Pathologists and Dietitians.
This position will be part of a team supporting older people to regain confidence, improve their function and remain safely at home after a hospital admission. The Program has a meaningful focus on reablement and person-centred care.
Functions of this role include reviewing referrals, travel within the LHD, completing discipline-specific delegated tasks, liaising with clients and families, documenting interventions and working with coordinators and other clinicians to support discharge planning and goal achievement.
Ongoing support will be provided through the Allied Health Team Leader, clinical supervision, multidisciplinary case conferences, peer support and opportunities for professional development.
