The Role
We are looking for an experienced Registered or Enrolled Nurse to join our Port Macquarie Medical Centre on a full-time basis and become a strong advocate for preventative healthcare within the local community.
Working closely with GPs, nurses, specialists and allied health professionals, you will identify patients at risk of chronic disease and support those living with ongoing health conditions. Through personalised care planning, education and regular monitoring, you will empower patients to take greater ownership of their health and achieve positive, sustainable outcomes.
Previous Chronic Disease Management experience is highly regarded; however, a friendly, proactive and supportive approach that complements our practice culture is equally important.
The Tasks
In this varied and rewarding role, you will:
- Prepare GP Management Plans and Team Care Arrangements to support patients in managing chronic conditions.
- Conduct patient-centred health assessments to assist with the early identification and prevention of chronic disease.
- Work with patients to establish meaningful and achievable SMART health goals.
- Coach patients to adopt positive and sustainable health behaviours.
- Connect patients with appropriate allied health professionals, specialists and multidisciplinary services.
- Coordinate holistic patient care in partnership with GPs and the broader clinical team.
- Promote preventative healthcare and Chronic Disease Management services throughout the practice.
- Identify eligible patients and encourage first-time participation in care planning.
- Provide ongoing monitoring and ensure care plans are reviewed regularly.
- Maintain accurate clinical documentation and complete administration relating to CDM appointments.
- Ensure services comply with Medicare requirements and ForHealth clinical standards.
- Support team members in identifying patients who may benefit from preventative health services.
- Contribute to the ongoing growth and success of the centre’s Chronic Disease Management program.
The Successful Candidate
We are looking for a nurse who is passionate about preventative healthcare and committed to working as part of a collaborative clinical team. You will bring:
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
- Previous experience in Chronic Disease Management, care planning or health assessments.
- Experience within a general practice or primary healthcare environment is highly regarded.
- Knowledge of Medicare requirements relating to care plans and health assessments.
- Strong organisational and time-management skills.
- Excellent written and verbal communication skills.
- The ability to motivate patients and encourage them to take ownership of their health.
- A proactive, adaptable and team-focused approach.
- A warm, personable manner and commitment to delivering an excellent patient experience.
