Primary objective
Territory Palliative Care (TPC) provides palliative care services to the entire Top End, including both rural/remote and urban areas. Your
role will be split between the three facets of our service: the Royal Darwin Hospital (RDH) inpatient consultation service, the Hospice (our
inpatient Palliative Care unit), and our community service. The case mix is diverse and unique with 40% of referrals being Indigenous
patients and 40% coming from rural/remote communities. The ratio of malignant to non-malignant cases is approximately 65:35.
Context statement
TPC is a small, multidisciplinary team, covering a large, complex geographical area. You will work closely with all members of this team.
Our aim is to impart you with skills and knowledge to help improve the standard of end of life care provided to all Territorians.
This role may suit a GP Registrar in training (pending re-accreditation with ACRRM and/or RACGP as required), a post fellowship general
practitioner, or any other trainee looking to upskill in the area of specialist palliative care. We are happy to support motivated candidates
to complete the Royal Australasian College of Physicians’ Clinical Foundation in Palliative Medicine during this posting.
Key duties and responsibilities
1. RDH inpatient consultation service: You will review patients referred from all inpatient teams. This is often the first encounter patients
and families have with TPC. You will work closely with a palliative care Clinical Nurse Consultant. We advise on multiple symptom
issues and facilitate/encourage discussions around goals of care and end of life preferences. We frequently help to coordinate return
of patients to country/community for end of life care. In the hospital consult role, you will also have the opportunity to attend MDT
meetings (cancer, renal) and see the decision-making processes that occur within a tertiary centre.
2. TPC Hospice: This is a 12 bed specialist inpatient facility. Your role will include admitting new patients and providing high quality
holistic and multidisciplinary care to patients with terminal illnesses. You will be closely supervised and guided by an experienced
palliative care team and partake in regular ward rounds, multidisciplinary team meetings, discharge coordination and family meetings.
3. Community: You will work closely with community palliative care case managers and community nurses. You will see many new clients
on home visits, in weekly outpatient clinics or via telehealth. During these visits, you will formulate care plans, discuss goals of care
and assist in creating advance care plans. You will liaise closely with community medical staff in Darwin and remote communities in
order to coordinate the best possible end of life care for patients and their families who wish to die at home/in community.
4. After-hours on-call: Shared between Registrars in week-long blocks on a 1:3 roster. The on-call doctor performs weekend ward rounds
in the Hospice. A consultant (second on-call) and nurse manager are also on-call to provide guidance.
5. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in
order to ensure high quality, safe services and workplaces.
Selection criteria
Essential
1. Eligible for general and/or specialist registration with the Medical Board of Australia.
2. Excellent interpersonal, oral and written communication skills and the ability to interact positively in a cross-cultural work environment,
as a team member.
3. An interest in and a commitment to improving Indigenous health.
