Rumbalara Aboriginal Co-Operative – Positive Ageing & Disability Services is currently recruiting for the following vacancies:

2x Care Manager (Case Management) - Shepparton site

2x Care Manager (Case Management) - Swan Hill site

Full-time | 38 hours per week | Fixed-term (24-month contract from the commencement date)

About the Role

This is a community-based role (not facility or residential care).

The Care Manager coordinates and manages services for participants under the Support at Home Program, working in partnership with clients to identify their goals, support needs, and preferences.

The role ensures services are culturally safe, person-centred, and aligned with program guidelines, organisational policies, and aged care quality standards, supporting clients to live safely and independently at home.

About You

Qualifications and Registrations:

Qualification in Aged Care, Community Services, Social Work, Case Management, or a related discipline, or equivalent industry experience.

Knowledge and Skills:

Experience supporting older people to live independently in the community.
Knowledge of aged care programs (Support at Home, Home Care Packages, CHSP) and brokerage models.
Understanding of person-centred care and consumer choice principles.
Awareness of the health, social, and cultural needs of Aboriginal and Torres Strait Islander Elders and communities.
Ability to assess client needs and develop coordinated support plans.
Experience using client management systems and maintaining accurate records.
Strong communication, organisational, and interpersonal skills.
Ability to work independently and within a multidisciplinary team.

Competitive pay and conditions under the SCHADS Award: Level 3 to 4 (based on experience and qualifications).
Salary packaging of $15,900 available for Part- or Full-time employees to eligible employees.
