About the Opportunity
Make a meaningful difference to patients transitioning from hospital back into their homes and communities as a Registered Nurse with the Transitional Aged Care Program (TACP).
Based within the Lower Mid North Coast (LMNC), our TACP provides care to patients following a hospital stay, supporting them to regain function, mobility and independence while working with patients and their families to establish ongoing care arrangements.
The program is based in Taree, with the successful applicant having the flexibility to work from Taree, Forster or Gloucester Community Health, depending on their geographical location. Our service provides coverage across the LMNC, including the outlying areas of Gloucester, Tea Gardens and Johns River, offering a varied and rewarding community nursing experience. You will be supported by the Nurse Unit Manager for Aged Care Assessment Services and TACP, Aged Care Service Manager and dedicated administrative support, with appropriate training and ongoing guidance provided to help you develop confidence and capability in the TACP role.
What You Will Be Doing
As a Registered Nurse within TACP, you will provide person-centred care to patients in the community following hospital discharge, helping them achieve their individual health, mobility and independence goals.
Your responsibilities will include:

Responding to referrals and completing timely intake assessments for patients in hospital.

Providing ongoing case management and coordinating patient care throughout their TACP journey.

Supporting patients to regain functionality, mobility and independence following hospitalisation.

Working with patients, families and carers to identify goals and develop appropriate care strategies.

Coordinating and finalising ongoing care arrangements to support a safe and sustainable transition from TACP.

Monitoring patient progress and adapting care plans in response to changing needs.

Providing clinical support and leadership within the multidisciplinary team.

Collaborating with Registered Nurses, Enrolled Nurses, Allied Health professionals and Domestic Support Services.

Working across the LMNC to provide responsive and effective community-based care.
About you
Our ideal candidate is a motivated and confident Registered Nurse who enjoys working in the community and is passionate about supporting patients to regain their independence following hospitalisation.
You will be able to take initiative when working autonomously in the community while knowing when to seek support and collaborate with colleagues. You will demonstrate strong interpersonal and communication skills and be confident consulting with a broad multidisciplinary team to deliver a professional, coordinated and person-centred service.

Current registration as a Registered Nurse with AHPRA.

Additional Information
- Based on a minimum 18-month commitment to the position or the agreed duration of your contract. All payments to part-time and contract employees will be made on a pro-rata basis in accordance with the terms of your contract, including any approved extensions.
