About the role:
An exciting opportunity is available for an experienced mental health clinician to join the Intensive Support Program (ISP) within Western Health’s Outer Community Mental Health Team.
This is a full-time position working Monday to Friday, offering regular weekday hours while providing continuity of care to consumers with complex mental health needs.
The Outer Community Team provides specialist mental health care across the diverse and rapidly growing communities of Melton and Brimbank. You will join a supportive multidisciplinary team comprising nursing, occupational therapy, social work, psychology, medical and lived experience disciplines.
As a Senior ISP Clinician, you will provide advanced, recovery-oriented assessment, intervention and care coordination to an allocated caseload of consumers with complex mental health and psychosocial needs. You will work collaboratively with consumers, families and carers to develop individualised care plans and provide intensive community-based support, including outreach, home visits and flexible engagement.
You will also play an important role in supporting the clinical capability and safe operation of the ISP. This includes providing senior clinical guidance to colleagues, assisting with complex decision-making and risk management, supporting junior clinicians and students, and contributing to safe service flow during periods of high demand or staff leave.
ISP is a small, cohesive team where collaboration, shared clinical decision-making and an integrated approach to care are central to achieving positive outcomes.
Key responsibilities:
- Providing advanced assessment, formulation, intervention and care coordination.
- Managing an allocated caseload of consumers with complex and high-intensity needs.
- Delivering flexible community-based care, including outreach and assertive follow-up.
- Identifying, assessing and responding to clinical risk.
- Coordinating care with psychiatrists, multidisciplinary colleagues and external service providers.
- Providing senior clinical guidance and support to clinicians and students.
- Supporting clinical handovers, case discussions and safe service flow.
- Maintaining accurate, comprehensive and timely clinical documentation.
- Contributing to clinical governance, reflective practice and quality improvement.
- Practising in accordance with the Mental Health and Wellbeing Act 2022 and relevant professional and organisational standards.
About you:
To be successful in this role, you will have:
- Relevant qualifications and current registration or professional eligibility in nursing, occupational therapy, social work or psychology.
- A minimum of five years’ public mental health experience for nursing applicants or seven years for allied health applicants.
- Advanced skills in assessment, formulation, intervention and recovery-oriented care.
- Experience supporting consumers with complex, high-risk and multifaceted needs.
- Strong clinical judgement and confidence in assessing, managing and escalating risk.
- A comprehensive understanding of the Mental Health and Wellbeing Act 2022.
- Experience providing clinical guidance, mentoring or support to colleagues or students.
- Strong communication, collaboration and documentation skills.
- The ability to manage competing demands within a community-based service.
- A current Victorian driver’s licence and the ability to drive Western Health vehicles.
