Full Job Description

As a Mental Health Practitioner with PLANZZ First Nations Health, you will work alongside people to understand their story, strengths, needs and recovery goals, providing practical, respectful and person-led support that responds to the whole person.

Your work will include comprehensive biopsychosocial assessment, recovery planning, psychosocial support, service navigation and participation in therapeutic group programs. You will support First Nations and non-First Nations people, bringing a strong commitment to culturally safe, trauma-informed, recovery-oriented and strengths-based practice.

You will work with people who may be experiencing emotional distress, suicidal ideation, mental health challenges and complex life circumstances involving housing, domestic and family violence, substance use, employment, financial pressures or physical health.

No two days will be the same. Your role may include:

Conducting comprehensive, trauma-informed biopsychosocial assessments
Developing and reviewing person-led recovery and support plans
Providing immediate, appropriate support to people experiencing acute distress, including suicidal ideation
Supporting people to build confidence, independence, capacity and practical life skills
Connecting people with clinical, housing, domestic and family violence, employment, financial, alcohol and other drug and community services
Supporting applications for the NDIS, Disability Support Pension, social housing and Victim Assist
Co-facilitating therapeutic and psychosocial groups, including DBT skills, art therapy and walking groups
Working alongside Peer Service Guides within the Centre’s welcoming drop-in environment
Providing outreach support in people’s homes and communities across Redcliffe and the Southern Moreton Bay region
Working collaboratively with families, carers, health professionals, community organisations and other service providers to create coordinated and practical pathways of support

At PLANZZ First Nations Health, we see effective mental health support as relational. It means listening first, understanding the person in the context of their family, community and circumstances, and working alongside them to identify pathways that strengthen wellbeing, independence and connection.

The Centre provides services across extended hours, and some Saturday work will be required.

Pay: $107,000.00 – $108,000.00 per year
