Role description
An exciting leadership opportunity exists for an experienced Registered Nurse to join Dhelkaya Health as the Community Nursing Clinical Coordinator. Reporting to the Community Nurse Unit Manager, this role provides operational and clinical leadership to the District Nursing and Community Palliative Care team, supporting the delivery of high-quality, person-centred services across the Mount Alexander Shire.
Working within a seven-day, extended-hours service, you will coordinate clinical operations including triage and intake, client assessment, care planning, workload management, risk management, discharge planning and quality improvement initiatives. You will play a pivotal role in supporting staff, fostering clinical excellence and ensuring the delivery of responsive, evidence-based care in the community.
Key Responsibilities
- Provide operational and clinical leadership to the District Nursing and Community Palliative Care service.
- Coordinate referrals, triage, intake assessments and service delivery across the community nursing program.
- Support comprehensive client assessments, care planning and targeted clinical interventions.
- Lead case conferences and facilitate collaborative multidisciplinary care.
- Oversee day-to-day service operations including rostering, workload allocation and risk management.
- Provide clinical consultation, support and supervision to community nursing staff.
- Lead quality improvement initiatives, accreditation activities and service development projects.
- Partner with key stakeholders across community, rehabilitation, palliative care and at-home services to ensure coordinated care outcomes.
- Support workforce development, staff wellbeing and positive team culture.
About you
To be successful in this role, you will possess:
- Bachelor of Nursing and current AHPRA registration.
- Extensive nursing experience, ideally with a minimum of five years' experience in community nursing or a related field.
- Significant clinical experience working within community nursing services.
- Strong leadership skills with experience leading teams through change and service improvement initiatives.
- Demonstrated knowledge of the health needs of older people, people with disabilities and their carers.
- Excellent communication, stakeholder engagement and conflict resolution skills.
- Ability to work autonomously, exercise initiative and develop innovative solutions.
- Strong organisational, time management and information technology skills.
- Postgraduate qualifications in a relevant clinical or leadership discipline will be highly regarded.
