What you’ll do

Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.

Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan

Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.

Social time and participation: Bring meaning, fun, and community access aligned with interests.

Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.

Cultural humility: Respect backgrounds, beliefs, and ways of being

Additional competencies required: Manual Handling, Acquired Brain Injury, Medication Administration, Changed Behaviour, Mealtime management, Complex Bowel Care, Severe Dysphagia Management
