About Us

Care Connector (Holistic Care Solutions) is a clinician-led provider delivering holistic, person-centred care across the community.

We specialise in complex care and support participants to live safely and independently in their homes. We are a small, values-driven team focused on quality care, strong clinical support, and meaningful community-based outcomes.

About the Role

We are seeking experienced Disability Support Workers available for morning and afternoon shifts (800-1400 and 1400-2200) for current and upcoming opportunities across the Murwillumbah and the Norther NSW Region.

This is not high-volume agency work, you will:

Work with consistent participants
Support complex and high-intensity needs
Be supported by experienced clinicians
Deliver care in participants’ homes and community settings

Key Responsibilities

Deliver safe, person-centred care to participants with complex needs
Assist with personal care, daily living and community access
Follow care plans and positive behaviour support strategies
Monitor and report changes in client condition
Complete documentation (progress notes, incident reports)
Build strong, professional and respectful relationships with clients

What We’re Looking For

For Support Workers:

Certificate III/IV in Individual Support (or equivalent experience)
Experience supporting participants with complex needs (desirable)

For All Applicants:

A current driver’s licence and reliable vehicle
Current National Police Check, or willingness to obtain one
Must have work rights in Australia
Previous experience in community care highly regarded
Experience or interest in complex care, disability, or community support
Strong communication and a calm, professional approach
Current First Aid & CPR
NDIS Worker Screening Check + Blue Card
Availability for flexible shifts (including weekends if required)
