About the role
As a community Dietitian, you will work with clients to provide nutrition assessment and planning of dietary needs, including food service management and intervention programs to provide your clients with a holistic and integrated care to enhance health and wellbeing.
In this role you will be…

Ensuring appropriate delivery of quality Dietetic services tailored to the individual needs of the client and using a person-centred approach

Conducting assessments and providing interventions to patients and clients in their own home

Engaging with other Allied Health Professionals, GP’s and other Health Care Professionals to provide effective patient care

Writing reports such as initial reports, progress and final reports and treatment management plans which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers

Able to reflect our practice values of Care, Respect, Integrity and Service Excellence in all that you do.

Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers.

Bachelor of Nutrition and Dietetics or equivalent that meets the requirements for full or part time membership of the Dietitians Australia (New graduates welcome to apply)

Ability to work within a multi-disciplinary team; and ability to work autonomously in the community

An enjoyment of working with the older adult and adult disability demographics

Full working rights in Australia
-
Clean driver licence, access to own vehicle and comprehensive car insurance.

Current Working with Children Check and NDIS Workers Screening Clearance, or able to obtain.
-
Willing to undergo a National Criminal History check upon successful application.
