Our team is seeking for an experienced registered nurse (with AHPRA registration) to deliver goal-directed, early intervention tele-health services including care planning, health coaching, and support coordination with a highly engaged patient population. We are looking for casual employee. You will have the opportunity to work flexibly as the role can be hybrid working arrangements.

Job Description
Tasks include:

Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals.

Undertake coordination of supports and wellbeing services.

Provide initial early intervention Triage with clients, supporting both physical and mental health services.

Undertaking of relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as a treatment plans or other documentation as required.
Qualifications
Requirements/Qualifications

Experienced Nurse with full registration.

Hold a current and valid Working With Children Check (WWCC)

Evidence of completion of First Aid (level 2) or in circumstances where staff have obtained “Basic Life Support” training within a hospital setting

Mental Health support service experience highly desirable.

Caring and empathic nature.
-
Experience care coordination or occupational rehabilitation.
-
Well-developed written and verbal communication skills.
-
Good IT and time management skills.

Strong customer service focus.
